RESUMO

Cariprazine is an atypical antipsychotic that has D2 and D3 partial agonism properties in addition to the usual 5-HT2A receptor antagonist action of second-generation antipsychotics. It has a distinctly higher affinity for D3 receptors, which is 10-fold higher than for D2 receptors. Cariprazine is also a 5-HT1A partial agonist, with a potential antidepressant effect. Cariprazine has been approved for treatment of both positive and negative symptoms of schizophrenia and for treatment of bipolar disorder. It could potentially be used in depression as an add-on treatment. There are few data reporting effectiveness of cariprazine in the broader spectrum of psychosis. In this paper, the authors report three cases where cariprazine was used in the treatment of psychotic conditions other than schizophrenia, namely a first episode psychosis, a case of delusional disorder, and a case of a patient with borderline personality disorder and psychotic symptoms. The authors suggest that cariprazine may be effective in the treatment of psychosis in a broader sense and should be considered a first-line treatment option.

RESUMO

Euclid is a European Space Agency medium-class mission selected for launch in 2020 within the cosmic vision 2015-2025 program. The main goal of Euclid is to understand the origin of the accelerated expansion of the universe. Euclid will explore the expansion history of the universe and the evolution of cosmic structures by measuring shapes and red-shifts of galaxies as well as the distribution of clusters of galaxies over a large fraction of the sky. Although the main driver for Euclid is the nature of dark energy, Euclid science covers a vast range of topics, from cosmology to galaxy evolution to planetary research. In this review we focus on cosmology and fundamental physics, with a strong emphasis on science beyond the current standard models. We discuss five broad topics: dark energy and modified gravity, dark matter, initial conditions, basic assumptions and questions of methodology in the data analysis. This review has been planned and carried out within Euclid's Theory Working Group and is meant to provide a guide to the scientific themes that will underlie the activity of the group during the preparation of the Euclid mission.

We present exact kinematic consistency relations for cosmological structures that do not vanish at equal times and can thus be measured in surveys. These rely on cross correlations between the density and velocity, or momentum, fields. Indeed, the uniform transport of small-scale structures by long-wavelength modes, which cannot be detected at equal times by looking at density correlations only, gives rise to a shift in the amplitude of the velocity field that could be measured. These consistency relations only rely on the weak equivalence principle and Gaussian initial conditions. They remain valid in the nonlinear regime and for biased galaxy fields. They can be used to constrain nonstandard cosmological scenarios or the large-scale galaxy bias.

Several extensions of general relativity and high energy physics include scalar fields as extra degrees of freedom. In the search for predictions in the nonlinear regime of cosmological evolution, the community makes use of numerical simulations in which the quasistatic limit is assumed when solving the equation of motion of the scalar field. In this Letter, we propose a method to solve the full equations of motion for scalar degrees of freedom coupled to matter. We run cosmological simulations which track the full time and space evolution of the scalar field, and find striking differences with respect to the commonly used quasistatic approximation. This novel procedure reveals new physical properties of the scalar field and uncovers concealed astrophysical phenomena which were hidden in the old approach.

RESUMO

Scalar fields are crucial components in high energy physics and extensions of general relativity. The fact that they are not observed in the Solar System may be due to a mechanism which screens their presence in high dense regions. We show how observations of the ellipticity of galaxy clusters can discriminate between models with and without scalar fields and even between different screening mechanisms. Using current x-ray observations we put novel constraints on the different models.

It is shown that extensions to general relativity, which introduce a strongly coupled scalar field, can be viable if the interaction has a nonconformal form. Such disformal coupling depends upon the gradients of the scalar field. Thus, if the field is locally static and smooth, the coupling becomes invisible in the Solar System: this is the disformal screening mechanism. A cosmological model is considered where the disformal coupling triggers the onset of accelerated expansion after a scaling matter era, giving a good fit to a wide range of background observational data. Moreover, the interaction leaves signatures in the formation of large-scale structure that can be used to probe such couplings.

Los tumores neuroendocrinos en la glándula mamaria, representan menos del 2 por ciento de las lesiones malignas que se presentan en la mama, un 30 por ciento pueden ser metastásicos, principalmente de tumores carcinoides intestinales. Se presenta el caso de una paciente femenina 64 años con el antecedente de carcinoma de mama izquierda pT1N1Mo Estadio II A, se le practicó en el año 2000 cirugía preservadora, recibió tratamiento sistémico y radioterapia, presentando recaída local, histológicamente y por inmuhistoquímica, como tumor neuroendocrino, en mayo de 2008 se le realiza mastectomía simple izquierda. Actualmente viva y sin enfermedad. Los tumores neuroendocrinos pueden presentarse en localizaciones extra intestinales. El diagnóstico debe realizarse por histología y confirmado con técnicas de inmunohistoquímica, son tumores de baja agresividad biológica, no se presentan síntomas sistémicos por liberación de hormonas como en otras localizaciones, y el tratamiento debe basarse en el estadio clínico de la enfermedad al momento del diagnóstico.


Neuroendocrines in the mammary gland tumors represent less than 2 percent of malignant injuries, which 30 percent can be source metastatic, mainly tumors intestinal carcinoid. The clinical of a female patient case 64 years old are presented with the antecedent of pT1N1Mo Stadium II A left breast carcinoma who was practiced in the year 2000 sparing surgery, received systemic therapy and radiotherapy, featuring local relapse, histological and inmuhistochemestry, as neuroendocrines, practicing it in May of 2008 mastectomy left, currently living and without disease tumor. Neuroendocrines tumors can occur in extra intestinal in lung, uterine and less common in the mammary gland neck locations. The diagnosis must be made by histology and confirmed with Immunohistochemistry techniques, are tumors of low biological aggression, not have systemic symptoms by release of hormones as in other locations, and treatment must be based on the clinical stage of the disease at the time of diagnosis.

We show that, as a result of nonlinear self-interactions, scalar field theories that couple to matter much more strongly than gravity are not only viable but could well be detected by a number of future experiments provided that they are properly designed to do so.

Se presenta la experiencia en el manejo de lesiones subclínicas de la glándula mamaria mediante biopsia radioquirúrgica en 31 pacientes asintomáticas entre febrero 1996 y abril 1999, en el Instituto de Clínicas y Urología Tamanaco, a quienes se les realizó mastectomía parcial previa localización con mamografía y/o ecosonograma mamario, utilizando marcajes con colorante y/o arpón. Resultados: el rango etario está comprendido entre los 35 y los 69 años, la relación de lesiones benignas/malignas es de 3,8:1, con un porcentaje de lesiones subclínicas malignas diagnósticas y tratadas del 25,80 por ciento. En el grupo etario comprendido entre los 50-59 años se presentó el mayor porcentaje de lesiones subclínicas malignas (16.12 por ciento), observándose que en el grupo de lesiones subclínicas tipo microcalcificaciones en el 42 por ciento de los casos fueron lesiones malignas de la glándula mamaria. Con respecto al tipo de tratamiento realizado en los casos de carcinoma ductal in situ y se realizaron cirugías preservadoras de la mama en los 3 pacientes, más radioterapia en 1 de los casos; en los pacientes con carcinoma ductal infiltrante el tratamiento fue tratamiento preservador de la mama que incluye mastectomía parcial oncológica, disección axilar y radioterapia a la glándula mamaria en todos los casos, observandose metástasis axilar en solamente 1 de ellos. Discusión: la utilización de la mamografía como método de pesquisa ha permitido el diagnóstico y tratamiento de un número mayor de lesiones malignas tempranas de la glándula mamaria, siendo la biopsia radioquirúrgica un procedimiento común, útil y frecuente en el manejo de éstas lesiones, con garantías de éxito en un equipo de trabajo multidisciplinario, mientras se logran desarrollar otros métodos menos invasivos en el diagnóstico de lesiones tanto benignas como malignas de la glándula mamaria

Se reporta el caso de un paciente de 33 años que presentó un cuadro de obstrucción colónica. La paciente fue intervenida quirúrgicamente con el hallazgo de una intususcepción colo-colónica. La lesión fue resecada y el estudio histopatológico reportó lipoma submucoso del colon. Se revisa la bibliografía y se menciona otros dos casos reportados en la literatura venezolana

Una lesión tumoral localizada en el dedo medio de la mano derecha de una paciente femena de 20 años de edad de características clínicas benigna y de un (1) año de evolución, fue resecada en 3 oportunidades por recidiva, haciéndose diagnóstico histopatológico de un "Neurotekeoma" con características macroscópicas de una lesión irregular, blanquecina, semifirme y mucoide, y características histológicas de cestas y cordones de células separadas por bandas fibrosas con gran pleomorfismo y con una matriz mucoide. A nuestro juicio las recidivas se debieron a lo difícil de establecer márgenes entre tejido tumoral y tejido sano durante los actos quirúrgicos haciendo difícil la resección total de la lesión, no encontramos publicaciones en la literatura de localización de estos tumores en la mano
